HOLMES ABSENCE VERIFICATION
Parents may use this electronic form to report a student's FULL DAY absence.
ICHAT
The District requires all volunteers to fill out an electronic ICHAT form in advance. Volunteers must submit a new new ICHAT form every school year.
MEDICATION AUTHORIZATION (PDF)
A completed Medication Authorization form must be on file in order for a student to use any type of over-the-counter or prescription medication at school.
